           COMMUNITIES IN SCHOOLS OF MADISON COUNTY  

                Jump Start 2013: Information Sheet

WHEN: We will be offering the same program on two different days. Students can register for only one of these sessions. To make this program available to the widest group of incoming sixth graders we have scheduled both sessions for the week just prior the first day of school. The sessions are limited to 40 students, and will be filled on a first come, first serve basis. The session dates are:

         Session 1 ~ August 12, 2013  Monday                   Session 2 ~ August 13,  2013  Tuesday

6pm. The extended hours are to allow for working parents to be able to have their child participate. This program is designed for a full day (8am -5pm) and we ask participants to be present for the full day

WHERE: Madison Middle School. We will start and end each day in the gym. Please look for signs.

SNACKS AND FOOD: We will be providing lunch, snacks and drinks throughout the day, all at no cost. Your student may choose to bring their own lunch which is fine.

CLOTHES: We will be walking and in the gym all day so please wear tennis shoes and play clothes.

****All Participants must be preregistered ***

tfield@madisonk12.net  Please include contact information (e-mail or phone), your name, your student’s name and the day you wish to participate (Monday 8/12 or Tuesday 8/13). You will be contacted to let you know that we have registered your child. Your child MUST have a signed release form on the day of the program. You may sign this when dropping off your student or print it from the web site and send it with your student on the day of the program. Hard copies of this information sheet and the release form are also available at Hot Springs, Mar Hill or Marshall Public Libraries and at Madison Middle School.
